Acne fulminans with systemic symptoms

A 16-year-old boy presented with an abrupt onset of painful haemorrhagic pustules, comedones, nodules and cysts, predominantly affecting the trunk, with accompanying fevers and arthralgias. A diagnosis of acne fulminans with systemic symptoms was made, and he was promptly commenced on oral prednisolone and erythromycin, followed by a longer duration of isotretinoin. Treatment successfully induced remission; however, he was left with severe keloidal and hypertrophic scarring.
