要旨 【背景】 ウェルニッケ脳症はチアミン欠乏によって生じる重度の神経障害である。一般にウェルニッケ脳症のリスクとして長期絶食やアルコール使用障害が知られているが，これらのリスクがない場合は鑑別が難しく，見逃されることも多い。 【症例】 フロセミドを長期間内服していた既往があり，アルコール使用障害はなく，わずか3日間の食事摂取不良期間を契機にウェルニッケ脳症を発症した症例を経験した。 【結語】 本症例では，長期フロセミド内服によるチアミンの排泄亢進が背景となり，短期間の食事摂取不良を契機にチアミン欠乏を来した可能性が考えられた。フロセミドは臨床上使用頻度が高く，ウェルニッケ脳症は診断・治療の遅れが予後に影響する重篤な疾患であるため，本症例を報告する。
