要旨【背景】急性硫酸銅中毒は稀であるが致死率の高い病態である。銅の排泄を目的に重症例では血漿交換が有効であったとの報告があるが，その有効性は確立していない。今回，急性硫酸銅中毒に対し血漿交換療法を行い，メトヘモグロビン血症および血管内溶血の速やかな改善を得た症例を経験したため報告する。【症例】 20歳代の男性，自殺目的に硫酸銅約30gを摂取した。その後まもなく腹痛と嘔気を自覚し救急要請した。来院時は血液検査に特記所見なく，CTと内視鏡検査から硫酸銅による腐食性食道炎の診断で入院とした。第2病日にメトヘモグロビン血症と血管内溶血所見，腎障害を認め急性硫酸銅中毒の診断でキレート療法を開始した。第3病日に腎障害に対し腎代替療法を開始した。キレート療法開始後も中毒症状は改善せず，第4病日に血漿交換療法を行った。第5病日にメトヘモグロビン濃度の著明な低下と血管内溶血の改善を認めた。その後は良好に経過し，腎障害も緩徐に改善し第41病日に腎代替療法を離脱した。第50病日に自宅退院した。【結語】急性硫酸銅中毒に対する確立された治療法はないが，キレート剤が無効な症例や臓器障害を伴う重症例において，血漿交換療法は考慮すべき治療選択肢の …
