The Maroon (Quilombola) population presents lower health indicators, which negatively impacts their quality of life. Compared to the urban population, these communities face more precarious conditions, largely due to geographic isolation, access limitations, and the low quality of services when available. As a result, residents are frequently unable to treat their illnesses locally and must travel to more distant health units. Therefore, this study aimed to analyze the health conditions of Maroon patients with diabetes mellitus in a Family Health Strategy unit in the municipality of São Luís, State of Maranhão. Basic, descriptive field research with a quantitative approach was conducted, alongside a literature review. The research field was the family health unit located in the Liberdade Urban Maroon Settlement (Quilombo Urbano Liberdade) in São Luís, Maranhão, with a non-probabilistic sample of one hundred and fifty Maroons. For data collection, an electronic questionnaire containing 25 questions was administered via the Google Forms platform. The results demonstrated that socioeconomic factors are important for adherence to pharmacological treatment; it was identified that the Maroon population has access to and support from the healthcare system, which is, however, limited by a low frequency of follow-up care; physical activity habits and cultural practices are geared toward alcohol consumption, which negatively impacts the health of this population, perpetuating inequalities and producing health inequities among Maroon patients.
