RÉSUMÉLa rétention vésicale complète est une urgence urologique fréquente dont la prise en charge repose sur un drainage vésical immédiat par sondage urétral ou cathétérisme sus-pubien, afin de prévenir les complications du haut appareil urinaire. Nous rapportons un cas inhabituel d'auto-sondage chez un adolescent de 15 ans, sans antécédent particulier, qui utilisait un fil électrique comme cathéter lors des épisodes de rétention complète d'urine. Admis pour une rétention aiguë, l'examen clinique a objectivé la présence d'un fil électrique faisant office de cathéter urétral, dont l'ablation a été réalisée au bloc opératoire sous anesthésie générale. L'exploration peropératoire a mis en évidence un rétrécissement acquis du col vésical responsable de l'obstruction sous-vésicale. Les suites opératoires ont été simples, avec ablation de la sonde urinaire à J7 postopératoire et instauration d'un traitement par alphabloquant. À six mois de recul, le patient présentait un bon jet mictionnel sans récidive. Ce cas illustre les dangers de l'auto-sondage non médicalisé et souligne l'importance de la sensibilisation des populations aux pathologies urologiques afin de prévenir les pratiques d'automédication par des matériaux inadéquats.ABSTRACTComplete urinary retention is a common urological emergency requiring immediate bladder drainage by urethral catheterization or suprapubic cystostomy to prevent upper urinary tract complications. We report an unusual case of self-catheterization in a 15-year-old adolescent with no significant medical history, who routinely used an electrical wire as a catheter during episodes of complete urinary retention. Admitted for acute retention, clinical examination revealed the presence of an electrical wire serving as a urethral catheter, which was removed under general anesthesia in the operating room. Intraoperative exploration revealed an acquired bladder neck stricture as the cause of the subvesical obstruction. Postoperative recovery was uneventful, with catheter removal on postoperative day 7 and initiation of alpha-blocker therapy. At six-month follow-up, the patient had a good urinary stream with no recurrence. This case illustrates the dangers of non-medicalized self-catheterization and underscores the importance of raising public awareness of urological conditions to prevent self-medication practices using inappropriate materials.
