RÉSUMÉIntroduction. Les traumatismes urétéraux iatrogènes constituent une complication redoutable de la chirurgie pelvienne, particulièrement en gynécologie où ils surviennent dans 50 % des cas, exposant à un risque d'insuffisance rénale et de morbidité sévère. En Côte d'Ivoire, les données sur cette pathologie restent limitées. Cette étude avait pour but de décrire les caractéristiques épidémiologiques, cliniques et thérapeutiques des traumatismes urétéraux iatrogènes en chirurgie gynécologique au CHU de Bouaké. Méthodes. Nous avons mené une étude rétrospective descriptive de dix ans (2011-2021) au service d'urologie du CHU de Bouaké, incluant 19 patientes ayant présenté un traumatisme urétéral iatrogène lors d'une intervention gynécologique. Les paramètres étudiés étaient les caractéristiques sociodémographiques, les circonstances du diagnostic, l'intervention chirurgicale causale, les modalités de la chirurgie reconstructive et les résultats évolutifs. Résultats. L'âge moyen était de 39 ans ; 58,8 % des patientes vivaient en zone rurale et 57,9 % étaient multipares. L'intervention causale était une hystérectomie d'urgence pour sauvetage maternel (42,1 %). Le délai moyen de consultation était de deux jours ; l'anurie était le principal signe révélateur (78,9 %). Une ligature bilatérale des uretères était observée dans 84,2 % des cas. La technique de réimplantation urétérovésicale selon Politano-Leadbetter était la plus utilisée (94,7 %). Les suites étaient simples dans 73,7 % des cas ; les complications incluaient une suppuration pariétale (15,8 %), une fistule vésico-vaginale (10,5 %) et un décès (5,3 %). Conclusion. Les traumatismes urétéraux iatrogènes en chirurgie gynécologique touchent des femmes jeunes, souvent issues de zones rurales, et surviennent principalement lors d'hystérectomies d'urgence. La réimplantation urétérovésicale permet une récupération satisfaisante dans la majorité des cas. Le renforcement de la prévention par la formation des chirurgiens et l'amélioration de l'accès à l'endourologie constituent des priorités.ABSTRACTIntroduction. In Côte d'Ivoire, data on this pathology remain limited. This study aimed to describe the epidemiological, clinical, and therapeutic characteristics of iatrogenic ureteral injuries in gynecological surgery at the Bouaké University Hospital. Methods. We conducted a retrospective descriptive study over ten years (2011-2021) in the urology department of Bouaké University Hospital, including 19 patients who presented with iatrogenic ureteral injury during gynecological surgery. The parameters studied were sociodemographic characteristics, diagnostic circumstances, causal surgical intervention, reconstructive surgery modalities, and outcomes. Results. Mean age was 39 years; 58.8% of patients lived in rural areas and 57.9% were multiparous. The causal intervention was emergency hysterectomy for maternal rescue (42.1%). Mean consultation delay was two days; anuria was the main presenting sign (78.9%). Bilateral ureteral ligation was observed in 84.2% of cases. Ureterovesical reimplantation using the Politano-Leadbetter technique was the most frequently used (94.7%). Postoperative outcomes were uncomplicated in 73.7% of cases; complications included parietal suppuration (15.8%), vesicovaginal fistula (10.5%), and one death (5.3%). Conclusion. Iatrogenic ureteral injuries in gynecological surgery affect young women, often from rural areas, and occur mainly during emergency hysterectomies. Ureterovesical reimplantation allows satisfactory recovery in the majority of cases. Strengthening prevention through surgeon training and improving access to endourology are priorities.
