Impact of Mavacamten in the treatment of hypertrophic cardiomyopathy

Introduction: Hypertrophic cardiomyopathy is a genetic disease characterized by left ventricular hypertrophy and, in some patients, left ventricular outflow tract obstruction. Mavacamten is a primary cardiac myosin inhibitor that reduces hypercontractility, decreases actin-myosin cross-bridge formation, and promotes more efficient diastolic and systolic function. Objective: Synthesize the scientific evidence regarding the impact of mavacamten on the treatment of hypertrophic cardiomyopathy. Methods: A comprehensive narrative literature review of recent publications identified in international scientific databases was conducted. Ten relevant publications were selected: five primary clinical studies and five complementary works focused on short- and medium-term efficacy, safety, and tolerability. Results: Mavacamten demonstrated a significant reduction in the left ventricular outflow tract gradient, improvement in NYHA functional class, a decrease in cardiac stress biomarkers (NT-proBNP and troponin I), and favorable changes in cardiac structural remodeling. Conclusions: Mavacamten constitutes an innovative and highly relevant pharmacological alternative, primarily for patients with symptomatic obstructive hypertrophic cardiomyopathy. Current evidence supports consistent clinical, symptomatic, and structural benefits, although future research with extended follow-up is needed to conclusively confirm its long-term efficacy and safety profile.
