Chronic pain is a health problem that can reduce quality of life and increase dependence on analgesic therapy. Acupuncture based on the regulation of Qi and Xue may serve as a complementary approach for chronic pain management; however, its clinical effectiveness and economic implications require measurable evaluation. This study aimed to analyze the effectiveness of acupuncture based on Qi and Xue regulation in reducing pain intensity, improving quality of life, decreasing analgesic use, influencing physiological parameters, and assessing cost-effectiveness among adults with chronic pain. A quasi-experimental design was conducted involving 80 adults aged 18–70 years with chronic pain lasting ≥3 months. Participants were divided into an intervention group (n=40) receiving acupuncture and a control group (n=40) receiving conventional treatment. Data were analyzed using paired t-tests, ANCOVA, Cohen’s d, and incremental cost-effectiveness ratio (ICER). The intervention group showed a reduction in VAS/NRS pain scores from 6.73 at baseline to 3.31 at week 8, compared with a 1.14-point reduction in the control group. The adjusted between-group difference was statistically significant (p
