The implementation of Electronic Medical Records (EMR) is a priority in the digital transformation of healthcare services in Indonesia. Although mandated by Minister of Health Regulation Number 24 of 2022, EMR implementation remains incomplete, particularly in primary healthcare facilities. This condition indicates that EMR implementation still faces challenges related to organizational resources. This study aimed to analyze the relationship between staff perceptions of 5M resources (Man, Money, Method, Material, and Machine) and EMR implementation. A quantitative approach with a cross-sectional design was used. The study population consisted of 110 healthcare workers and staff involved in EMR use, selected through total sampling. Data were collected using a validated and reliable questionnaire. Data analysis included univariate analysis, the Chi-Square test, and multiple logistic regression. The results showed that 62.7% of respondents had good EMR implementation. Most respondents had good perceptions of Man (58.2%), Money (57.3%), Method (66.4%), Material (56.4%), and Machine (67.3%) factors. Significant relationships were found between Man (p=
