Intraoperative and Early Postoperative Complications of Hip Arthroscopy

Abstract Background: Hip arthroscopy is widely used for treating various hip pathologies and is considered a safe joint preservation procedure. However, reported complication rate varies considerably, likely reflecting differences in the technique, traction protocols, and patient-related factors. Subjects and Methods: A retrospective review of 1000 consecutive hip arthroscopy surgical procedures was performed to evaluate intraoperative and early complications occurring within 2 weeks postoperatively. Procedures were performed in the supine position using traction against a perineal post. Surgical technique, its modifications over time, and the potential risk factors, causes, and management of complications were analyzed. Results: A total of 115 complications were identified in 732 male and 268 female patients with ages ranging from 5 months to 72 years. Traction-related complications were the most frequent. Pudendal nerve palsy ( n = 42) was the most common nerve palsy. Iatrogenic labral penetration ( n = 22) and femoral head scuffing ( n = 14) were the most common intra-articular complications. Other complications included instability ( n = 4), abdominal distension ( n = 4), instrument breakage ( n = 5), foot pain ( n = 12), one fracture, and one case of pulmonary edema. Scrotal, vulvar injuries, and one case of Fournier’s gangrene were among the perineal skin complications. No early complications were observed in the eight postless traction procedures. Conclusion: Traction- and technique-related complications are the predominant early complications following hip arthroscopy. Careful attention to traction methods and surgical technique may further reduce complications.
