Introduction: The absence of a tooth due to agenesis, accidents, or other pathologies indicates treatment alternatives for its replacement. One of the solutions for this clinical situation is auto transplantation, which is defined as the replacement of the lost tooth with another tooth from the same patient. Objective: To evaluate the success of dental auto transplantation in terms of the presence of the tooth without ankylosis or root resorption, periodontal and peri-radicular stability, and aesthetics over a period of 4 years. Case presentation: A 27-year-old male patient who suffered dentoalveolar trauma to tooth 21 and consulted for a change in color. His endodontic diagnosis was invasive external root resorption with a Patel 3Dp tomographic classification. As treatment, auto transplantation of tooth 28 was performed in the position of tooth 21, following surgical and tomographic planning with a 3D-printed replica of the tooth in order to reduce extraoral time and prevent resorption after auto transplantation. Dental characterization procedures with crown and gingival leveling were performed by oral rehabilitation and periodontics. Conclusion: Multidisciplinary planning in dental auto transplantation is predictable over 4 years, reducing risks such as root resorption, and is therefore a successful treatment option for tooth loss in the anterior region.
