Background: Obesity is an important risk factor for chronic mechanical low back pain (LBP) due to altered spinal biomechanics and reduced core stability. Exercise therapy such as back exercise and yoga is commonly used; however, comparative evidence in overweight and obese patients is limited. Aim(s): To compare the effectiveness of back exercise and yoga on pain intensity, trunk muscle strength, and functional disability in overweight and obese patients with chronic mechanical LBP. Materials and Methods: A randomized controlled trial was conducted involving overweight and obese patients with chronic mechanical LBP. Subjects were divided into back exercise and yoga groups. Both groups received standard aerobic exercise using an ergocycle, twice weekly for six weeks. The back exercise group performed structured back exercises, while the yoga group attended 45-minute yoga sessions twice weekly for six weeks. Outcomes included pain intensity (Visual Analogue Scale), trunk muscle strength (handheld dynamometer), and functional disability (Oswestry Disability Index). Results: After six weeks, no significant differences were found between groups. Reductions in pain intensity, increases in trunk muscle strength, and improvements in functional disability were comparable between the back exercise and yoga groups (p > 0.05). Conclusions: Back exercise and yoga have similar effectiveness in reducing pain, improving trunk muscle strength, and decreasing functional disability in overweight and obese patients with chronic mechanical low back pain.
