Introduction: Forklifts play an important part in supporting loading and unloading operations within the fleet. Based on the company’s 2024 near-miss records, they account for the highest proportion of incidents, with 9 near misses—representing 69%—mainly attributed to human error. These human errors among forklift operators are categorized as unsafe behaviors. Methods: The goals of this study is to present an analysis of the impact of communication and procedures, as well as risk perception, on driver behavior; to identify the variables with the most substantial influence; and to provide some suggestions based on the findings. A quantitative approach was implemented by using a Likert scale questionnaire to assess the variables. Data analysis employed Pearson’s correlation test, classical assumption testing, and multiple linear regression. Results: The regression results indicated that both communication and procedures (p-value = 0.006) and risk perception (p-value = 0.006) significantly affect driver behaviour. Conclusion: Based on these results, recommended actions include scheduling regular safety briefings, introducing 10-minute break periods, enforcing reward and punishment policies, increasing the frequency of safety talks, implementing hazard observation forms, displaying safety posters, and organising in-house training sessions.
