European Health Evidence

The European alternative to PubMed

EUVIMED is the European alternative to PubMed: a central, multilingual research platform for medicine, nursing, life sciences and healthcare. It brings together international and European literature sources, study registries, open-access full texts, citations and retraction notices in one search. Unlike pure bibliographic databases, EUVIMED supports the entire research process – from discovery and appraisal with LIVIA and CLARA to traceable evidence synthesis. European in focus, transparent, interoperable and designed for science and healthcare.

Transformational Clinical Matrix-Expanded: Metatheoretical Framework for Conceptualizing Dysfunctional Processes and Psychopathology

Vollständiger Abstract

Worum geht es in dieser Arbeit?

Complex psychopathology, particularly when associated with adverse developmental experiences and complex trauma, involves multiple cognitive, affective, behavioral, interpersonal, and neurobiological processes that may be difficult to organize within single-theoretical or symptom-based models. The present paper introduces the Transformational Clinical Matrix-Expanded (TCM-XP), a transdiagnostic and metatheoretical framework developed to organize these processes within a developmental and integrative perspective. The TCM-XP comprises seven interrelated domains: Adverse and Dysfunctional Experiences, Emotional Wounds, Dialectical Core Schemas, Affective Dysregulation, Coping and Dispositional Responses, Maladaptive Patterns, and Complex Neural Network Dysfunctions. These domains are proposed to operate recursively rather than independently, suggesting that adverse relational and contextual experiences may contribute to the frustration of core biopsychosocial pressures, affectively encoded emotional wounds, polarized cognitive-affective representations, affective dysregulation, and recurrent coping and interpersonal patterns. The model also considers the possible contribution of large-scale brain networks and related neurobiological systems without reducing psychological functioning to neural mechanisms. By integrating developmental psychopathology, trauma theory, clinical psychology, personality perspectives, and affective neuroscience, TCM-XP provides a conceptual organization of complex psychological functioning that may inform assessment, case conceptualization, and clinical decision-making.
