PI-RADS category 3 lesions represent uncertainty and equivocal findings, and incorporating prostate-specific antigen density into a risk-based diagnostic pathway may optimize the balance between avoiding unnecessary prostate biopsies and reducing the number of missed clinically significant prostate cancers.
