Use of Intralipid Therapy and Therapeutic Plasma Exchange in the Management of Cholecalciferol Rodenticide Exposure in a Dog: A Case Report

ABSTRACT Objective To describe the presentation and outcome of a case of suspected cholecalciferol rodenticide intoxication managed with both intralipid emulsion (ILE) and therapeutic plasma exchange (TPE). Case Summary A 3‐year‐old, neutered female, crossbreed dog presented for acute‐onset vomiting and lethargy after suspected exposure to cholecalciferol rodenticide. Severe hypercalcemia was observed without other identifiable causes found upon investigation. Standard therapy for hypercalcemia was initiated, but rebound hypercalcemia led to consideration of alternative management options. Although ILE therapy initially showed promise, rebound hypercalcemia persisted. Ultimately, TPE was performed as a novel therapy and successfully reduced serum concentrations of vitamin D metabolites. Normocalcemia was eventually maintained, though the dog developed endocarditis that progressed to congestive heart failure and death. Blood cultures revealed the growth of Serratia marcescens . New or Unique Information Provided To the authors’ knowledge, this is the first report describing the use of both ILE and TPE in the management of cholecalciferol rodenticide intoxication in a dog. It is also the second reported case of S. marcescens causing endocarditis in a dog.
