Treatment Approaches for Pre‐Eruptive Intracoronal Resorption According to Lesion Grade and Pulp Involvement: A Systematic Review of Case Reports and Case Series

ABSTRACT Objectives Pre‐eruptive intracoronal resorption (PEIR) is a pathological condition lacking standardized treatment guidelines. This systematic review aims to synthesize published evidence on PEIR treatment strategies. Materials and Methods Five databases (PubMed, Web of Science, Scopus, Embase, and Dentistry & Oral Sciences Source) were searched according to a registered PROSPERO protocol (CRD420251018329, 28 March 2025). Eligible studies were case reports and case series describing PEIR treatment and follow‐up. Methodological appraisal used the Joanna Briggs Institute Critical Appraisal Checklists for Case Reports and Case Series. Results Forty‐six studies (35 case reports and 11 case series) were included. Treatment varied according to lesion size, eruption status, and pulpal involvement, ranging from observation/sealants (12 cases) to restoration (14), surgical exposure (12), vital pulp/endodontic therapy (9), and extraction (14). Conclusions Published case reports and case series describe a range of management approaches for PEIR, with variable clinical and radiographic outcomes. However, the available evidence does not permit reliable estimates of treatment success, failure, recurrence, or prognosis, nor comparison of the effectiveness of different management approaches. Early radiographic detection and careful follow‐up may facilitate timely intervention, but further multicenter studies are needed to establish evidence‐based management recommendations.
