Addressing Gaps in Supportive Care for Pediatric Cancer Patients in Africa: Insights from a Multinational Survey by the SIOP Africa

Abstract Background In Africa, delivering comprehensive supportive care is hindered by resource constraints, diverse healthcare systems, and cultural variations. To address these challenges, SIOP Africa has established supportive care Research working group. The primary objective of this group is to enhance supportive care practices and outcomes through collaborative research and capacity-building initiatives across the continent. Methods A survey was conducted to identify specific needs, gaps, and priorities in supportive care for pediatric oncology patients in Africa by targeting healthcare professionals working in diverse African settings. Results A survey conducted across 19 African countries, involving 46 respondents, revealed critical insights into the management of infectious complications in pediatric cancer patients. The respondents represented diverse roles, with 70% being pediatric oncologists. Among the surveyed institutions, 87% reported having a dedicated pediatric oncology unit, and 78% had a designated infection control team. Gaps were evident in protocols and expertise, with only 65% of institutions having protocols for managing infectious complications, 57% having trained healthcare staff, and 60% implementing infection prevention protocols. Furthermore, 40% lacked any training programs. In terms of diagnostic and treatment resources, 48% of institutions reported having an antimicrobial stewardship program, and 60% had access to antibiograms, but many lacked essential diagnostic tools, antibiotics, antifungal medications, and advanced medical facilities. Key gaps identified included the absence of standardized protocols, insufficient staff training, and limited access to necessary medications and diagnostic tools. To address these challenges, respondents emphasized the need for training programs to enhance healthcare professionals' skills, collaborations to share resources and improve care. Conclusion This survey underscores the critical need to address resource limitations, training deficiencies, and gaps in infection control practices to improve the outcomes for pediatric cancer patients in Africa. By focusing on capacity building, education, and collaboration, substantial progress can be made in managing infectious complications and overall quality of supportive care.
